Transportation

Transportation Services: The limited amount of public transportation in our vast seven-county region creates a huge obstacle for clients. In order to stay healthy, individuals must have access to medical treatment, support groups, food pantry visits and other vital services. HVCS provides more than 11,000 bus and taxi rides per year for clients affected by HIV/AIDS. These services are available to HVCS’ clients as well as to HIV-positive clients of other organizations.

Our Tri-County Ryan White Part A Transportation program (Westchester, Rockland and Putnam) can now reimburse Ryan White eligible clients who drive their own vehicles for medical visits and supportive services appointments. Clients are reimbursed for gas mileage/tolls/parking costs by completing a travel reimbursement voucher. Forms must be completed and signed by the client, and  expenses verified by case managers. Contact our Transportation program for more details.

Expanded transportation service for Health Home clients

HVCS is now offering its Health Home clients transportation service to help them get to emergency medical and government benefits appointments, as well as certain community or wellness destinations.

HVCS recognizes that individuals with complex health conditions often need connections to additional support that can help them stay healthy. The expanded service can help with transportation needs that medical providers or care managers recommend, such as:

  • emergency medical appointments (if Medicaid is unable to meet need)
  • housing or Dept. of Social Service entitlement appointments
  • food pantry visits (once or twice a month)
  • support groups
  • programs/classes to quit smoking, lose weight or eat more healthy (once or twice a month)
  • other essential services

This service is available for enrolled Health Home clients with no other alternative means of travel. The care manager will determine if the destination meets the eligibility criteria.

This service does not replace Medicaid transportation for medical or case management appointments.
