Self-Assessment: Alcohol & Substance Abuse

To give yourself a self-assessment on your alcohol usage, open and print out this PDF document. It includes instructions on how to score the assessment too. A score of 8 or more indicates a strong likelihood of hazardous or harmful alcohol consumption. If you do score an 8 or more, or if you have any question about your or someone else’s alcohol use, please contact your local HVCS office for assistance and referrals.

To help you determine if you have a problem with alcohol, open and print out this PDF document. The consequences of your drinking can be a strong indicator if you have a problem.

A score of 5 or more may indicate problem drinking and warrants further evaluation. If you do score a 5 or more, or if you have any question about your or someone else’s alcohol use, please contact your local HVCS office for assistance and referrals.
